How might a Spanish teacher in a Spanish class ask her students to please answer a question?

Would it be, "(Student's name), conteste por favor.”
Yes, that's fine.

But e.g. in Spain (depending on the institution and the students' age), you could use the form 'tú'; I'd say for example: X, ¿podrías/puedes responder/contestar (tú a la pregunta), por favor? (Quite formal) ¿Contestas/Respondes tú, X, por favor? (A bit more direct and less formal) X, responde tú (a la pregunta), por favor.

I'm too polite and would usually add 'por favor', but I don't think that it's actually necessary to do it all the time in this context.

Well, in Spanish from Spain, you would normally use 'tú' in that case -at least I would (e.g. X, ¿puedes (tú) contestar/responder a la pregunta, por favor?), but as I said, it may well depend on the region and the school.
So if it was a highschool class in America for first or second year students, what would be the best option?

I would say:
"(Student's name), ¿puedes contestar a la pregunta, por favor?” or "(Student's name), contesta a la pregunta, por favor.
