I would like to simplify the below sentence.

Mr Chan was accompanied by Mrs Chan to come to our centre for opening an account at our service counter.

1. Mr Chan wanted to create an account being accompanied by Mrs Chan.
2. Mr Chan wanted to create an account which accompanied by Mrs Chan.
3. Mr Chan wanted to create an account which was accompanied by Mrs Chan.

Which one is better?

In all of your sentences, Mrs. Chan seems to be accompanying the account rather than Mr. Chan. Try:
Mr. Chan, accompanied by Mrs. Chan, came to open an account.
May I change the sentence to (Mr Chan came to open an account, accompanied by Mrs Chan.)
