No, the doctor wouldn't yet know the test result.

"If you are, God forbid, HIV positive......." means "God forbid that this should be the case".

So the doctor is awaiting the test result and praying that it won't be positive.

The sentence is incorrect though, HIV does not equate to AIDS.
This is a relic of the subjunctive. "May God forbid that this happens".

I think "God forbid" is said when someone is going to talk about a "if" situation. And the situation is a bad one for the listener. To not "brings bad luck" to the listener, the speaker would say "God forbid". It does not mean the speaker is superstitious, rather, a showing of good manners to the listener. That's how I see it.
