Untersuchungsbestätigung (There is no such expression as "to whom it may concern" in German, at least not commonly used for this kind of certificate)

Hiermit wird bestätigt, dass ich Frau XXXX am heutigen Tage untersucht habe. (Alternatively: Hiermit bestätige ich, Frau XXXX am heutigen Tage untersucht zu haben.) Sie ist körperlich und geistig gesund. Es wurden keinerlei Krankheiten festgestellt.

That's correct--but it looks somehow odd to me on a certifikate like that and is really unusual as a salutation in German. An alternative would be:

Zur Vorlage bei/für (name of the authority/employer/whoever might be concerned) 👁 Smile :)
