I've done testing in game and after 25w03a (including 1.21.5+), players die from a fall of exactly 23 blocks or more. This is lower than the distance described in the damage page: "a fall of at least 23.5 blocks is required to kill the player". Fall damage reduction in lava

Per my reddit comment, in Revision 2024-February-20 23:46 the entry for fall damage into lava was changed.
Old:
"Being in lava reduces fall distance by half each tick."
New:
"Being in lava reduces fall distance by half a block each game tick."
Changing from "by half" to "by half a block" represents a very big difference in meaning and game effect (despite the indubitably rare resultant play difference... y'know, due to the subsequent burning in lava). The former implies that Δfall reduces by 50% each tick, while the latter is Δfall changes -0.5m per tick. Without a citation it's unclear why this change was made. As the author, User:Simanelix hopefully has some insight into the change. Or maybe someone familiar with the minecraft source code can confirm which is the correct game effect and text version to preserve.

Or I might have misinterpreted the original text and thought it means half a block each tick, even though that would not make sense in gameplay in my opinion. In either case it was just a mistake.
